The St Charles office is currently seeking like-minded, values-driven individuals for Team Leader positions in our residential program. A Team Leader is responsible for coordinating comprehensive services for individuals with intellectual disabilities in a shared living environment. The Team Leader works closely with companions and direct support professionals to ensure that each individual supported is living the life that they want to live.

The job duties of this professional level position include, but are not limited to:

  • Actively participate in the person centered planning process, including but not limited to: attending all planning meetings, providing input, coordinating the development of training plans, ensuring all necessary documentation is completed, reviewing progress notes and daily notes, monitoring for the implementation of the plan, completing monthly reviews, and communication with service coordinators.

  • Coordination and oversight of medical appointments, including attending appointments if necessary. Communication with nurses and doctors about consumer's medical needs.

  • Face to face visits with individuals in their homes, ensuring that each individual's rights are respected and protected, personal plan objectives are being implemented.

  • Maintain positive communication and relationships with family members, guardians, staff, and Regional Office personnel about policies, issues, and/or concerns.

  • Prepare and submit applicable reports and documentation related to individuals supported by RHD.

  • Provide supervision and leadership to companions and direct support professionals.

  • Participate in hiring and training of key personnel, including coordinating ongoing training needed to assist staff in fulfilling responsibilities.

  • Develop staff schedules and ensure appropriate staff coverage is maintained within each individual home.

  • Be available and responsive to companions and direct support staff in emergency and crisis situations, including on call duties. Weekend on-call is rotated between team leaders approximately one time per month.

Candidates should possess the following traits and skills:

  • Knowledge of the Kansas City community and local resources

  • Ability to develop partnerships with family members and other provider organizations

  • Experience and working knowledge of person centered planning

  • Compassionate/Understanding

  • Minimum Qualifications/Requirements:*

  • Bachelors Degree in a Human Services field, including but not limited to sociology, special education, rehabilitation, nursing, counseling, and psychology.

  • At least one year's experience providing support to individuals with intellectual disabilities.

OR

  • 5 years experience providing support to individuals with intellectual disabilities.
